Sources and standards

What a source label means
“Official page read” means the contact appeared on an organization’s website or document during research. “Search or secondary source” means the record comes from an excerpt or another reference. “Needs confirmation” means the evidence is incomplete or older. “Official dataset read” means the contact was checked in the cited official structured directory. It does not mean every linked organization website was individually checked. A source check is not a test call. Each contact carries its own date.
Source currency
New additions and corrections are checked against the current official contact page or locator. A dated guide is used only when the current official site still identifies it as the applicable resource; its publication date stays separate from the source-check date. Conflicting or unsupported details stay flagged or are withheld pending review. A recent retrieval date alone does not prove that every fact on a website has been updated.
Individual facilities
Lab, pharmacy and HRSA health-center sites have separate location records, addresses and source links. Different source programs can refer to the same physical place. Counts describe location records and do not measure unique facilities. A corporate phone is never substituted for an unpublished branch phone. Shared numbers and appointment-specific numbers are labeled. Temporary closure notices, moved addresses, pharmacy hours and source conflicts stay attached to the location.
HRSA source records
HRSA sites are grouped by the exact publisher program ID. Active is the dataset status, not a verified open facility. Administrative, service-delivery, combined, mobile and seasonal roles retain their source labels. The dataset update date is separate from the check date and does not date each phone or address change. Registry addresses are not confirmed visitor or document-submission destinations. Operating hours per week do not establish daily opening or phone hours. Fourteen publisher-withheld site addresses remain blank; these site pages are excluded from indexing and the sitemap, and address corrections are not accepted. No parent address, coordinates or geocoding fills the gaps.
NANP phone candidates have structural formatting checks only. Basic Lookup evidence for 32 exact Pacific source rows checks range, length and prefix; neither establishes current ownership, service, reachability or extension delivery. Ambiguous, unsupported and structurally invalid numbers remain plain source text without dial links. Missing fax or email fields remain blank.
Coverage
The directory contains 4206 organization records, 1589 separately identified HRSA program records and 35144 contact records. Organization records and source program groups are counted separately; their names are not evidence of unique legal entities. It covers national and regional insurers, state programs and healthcare service organizations. Coverage is expanding; it is not an exhaustive list of every insurer, plan or facility.
How to choose a route
Confirm the legal plan or administrator, product, state, service and department. A parent-company contact may not handle a subsidiary’s claims. Mailing addresses and faxes have explicit purposes where researched. A blank field means unknown, not unavailable.
Published service commitments
A policy may promise a response within a stated time. We show that commitment separately with the exact product, policy and source. It is not an observed wait, an average, or a prediction for another phone line.
Published calling advice
Some insurers publish suggested calling windows or callback options. These are shown only for the stated contact and product, with the original publication date and source link. Older advice may no longer reflect current queues. Conflicting hours or menu options are flagged; a source check does not resolve them through a test call. This guidance never populates caller-report statistics.
Wait times, ratings and best times to call
We do not infer queue length from opening hours or manufacture ratings. Reports describe an individual caller’s experience and are moderated. Statistics use approved reports from the last 90 days, grouped by exact contact. At least five distinct contributors with answered calls are required before we show median and 90th-percentile waits. Abandoned and closed-line calls remain separate outcomes.
A suggested calling window requires at least two comparable weekday/time windows with five distinct contributors each, in the same reported time zone. The lowest observed median is described as an observation, never a guarantee. The window, sample size and period accompany the result. Contributors are distinguished by a signed browser cookie, not verified identity. Moderation checks for duplicate or suspicious submissions, but anonymous reports can still be biased. A small or self-selected sample may not represent your call.
Corrections and publication
Corrections stay private until an operator checks the cited official source. Approval does not automatically overwrite payer contacts. The source file is edited through a reviewed change, then the directory is rebuilt. Call reports never update a phone number or an IVR sequence.
Independence
DialClerk operates this directory to help healthcare offices and introduce its office calling tools. It is not an insurer and is not affiliated with listed organizations. Listing order is alphabetical; there are no paid ranking placements or invented testimonials.
