Directory privacy
Business contacts.
No patient records.
Practice inquiries
We collect your name, work email, practice, role, approximate weekly call volume and the directory page where you submitted the request. These records are encrypted in application storage and accessible to the deployment operator. Your consent permits an email response about your request, not automated calls, texts or unrelated marketing. The form does not send an email by itself.
Call reports and corrections
Reports collect a contact identifier, call date and time, time zone, wait duration, outcome and rating. Corrections collect the field, proposed public value and source URL. These go to a private moderation queue. We publish only aggregate approved call statistics after the sample thresholds are met; raw reports, names and emails are not published.
Abuse prevention and retention
The submission service sets a signed, first-party browser cookie lasting up to 180 days to recognize repeat submissions. A keyed hash of that browser identifier distinguishes contributors. We also retain a keyed hash of the request’s network address for rate limits and abuse review. The raw address is not stored by the directory service; infrastructure may maintain its own access logs. Directory records expire after 180 days. The system removes expired intake records when processing requests or through the operator maintenance command.
Remove your submission
After a submission, the confirmation displays a private removal code. Keep it to delete that record below. This code is not sent by email and should not be shared.
Do not submit sensitive information
Do not enter patient names, member IDs, claim numbers, diagnoses, medical records, credentials or payment details. This public directory is not a clinical service or a secure patient communication channel.
