A Practical Payer Call Checklist for Medical Office Staff
A good payer call ends with a record another staff member can act on. Use this checklist to prepare the conversation and distinguish a useful answer from an item that still needs work.
Start with the right department.
Find published phone, fax and email routes, with source dates and plan-specific details.
Find healthcare contactsBefore the call
Write a one-sentence objective. “Check the claim” is broad; “confirm whether the claim was received and what is blocking processing” gives the caller a specific job. Check the payer and department against your current source, then gather only the identity and account details required for that job.
- Confirm the payer, plan, and correct department.
- Have the authorized patient and provider identifiers available.
- List the exact questions in the order they should be asked.
- Decide which questions need a biller, coordinator, or clinician.
During the conversation
Separate answers from assumptions. If a representative gives a general policy answer, clarify whether it applies to the specific plan, service, provider, and date you asked about. Record unclear or unanswered items as unresolved rather than filling the gaps.
- Record the representative’s name and department.
- Capture the answer to each original question.
- Read back important identifiers and destinations.
- Ask what is missing, who should act, and when to follow up.
Before hanging up
A reference number and a practical callback route make a future conversation easier to continue. If the office needs to send something, confirm the recipient and destination; do not assume an address or fax number from an old record is still appropriate.
- Request the call reference number.
- Confirm any case or authorization identifier.
- Repeat the agreed next step and any date the representative provided.
- Identify questions the representative could not answer.
After the call
Save the reported outcome, the evidence, and the next action as separate information. A call completed successfully can still leave the task unresolved. Assign a person to the follow-up, and use the office’s existing system to track it.
- Keep the date, payer, representative, and reference together.
- Label statements as payer-reported until reconciled with the relevant evidence.
- Assign an owner and follow-up date for open items.
- Review the record before marking the underlying work complete.
