The inherited prescriber-fax value is withheld from the active fax field pending current intake confirmation. Its former plan FAQ now returns404. The current Costco Home Delivery FAQ publishes a different Pharmacy Member Service fax, 800-633-0334, which is not substituted as a new-prescription fax. Confirm the prescription-specific method with the pharmacy.

The current Costco Specialty page hosted by Lumicera directly publishes the phone, fax and email. The former prescription-fax label is narrowed to its established department-contact purpose. Email display/href capitalization differs only in case. An empty link to another phone has no established role and is excluded. No general Costco mail-order, warehouse, insurer coverage or corporate ownership relationship is inferred.

The current Home Delivery FAQ confirms the published agent hours and separately describes 24/7 self-service refill/order/status availability. Agent service is not claimed staffed 24/7. This route is separate from local warehouse pharmacies and Costco Specialty.

The currently linked general-enrollment form explicitly describes the Costco/Lumicera partnership.
