A denied pharmacy claim submitted to the Bridge BIN/PCN is required BEFORE the prescriber submits this request; this is not a requirement for a prior Part D denial. Form submission is restricted to prescribing clinicians. Conditions eligible for regular Part D coverage follow that plan's process. Temporary Medicare GLP-1 Bridge demonstration, July 1, 2026–December 31, 2027. Only eligible Part D beneficiaries and the current named formulations qualify: Foundayo tablets, Wegovy injection/HD/tablets and Zepbound KwikPen; single-dose Zepbound pens/vials excluded. Separate from commercial cards, manufacturer PAP and IRA negotiated-price rules. Eligibility and coverage must be confirmed; no price guarantee.
