Progressive's New Jersey Decision Point Review Plan says decision point review and precertification requests must be faxed to this number, using the uniform Attending Provider Treatment Plan form. New Jersey only. Auto medical payments/PIP: a claim number and the claim adjuster or policyholder identification are usually required; coverage is determined by the claim and policy (not by a member ID), and state PIP fee schedules and rules govern billing.
