Each reviewed brand page describes PAP assessment after Start Form submission, primarily for uninsured patients, and a separate commercial-insurance copay program. Government-sponsored coverage is excluded from copay support. The program makes individual eligibility determinations.

Use the patient reimbursement form with the requested receipt, EOB or billing statement. Commercial insurance and the form’s terms apply; administration costs are subject to location restrictions. This is reimbursement intake, not a PAP application or patient payment destination.
