NJ PIP medical bills

Allstate's New Jersey auto claims page: submit bills by eBill (Allstate NJ payor ID LU610), by mail to Allstate Insurance Co c/o Medlogix, by fax or by email. Government lienholders (Medicare, Medicaid, etc.) use a separate Dallas P.O. box listed on the same page. Auto medical payments/PIP: a claim number and the claim adjuster or policyholder identification are usually required; coverage is determined by the claim and policy (not by a member ID), and state PIP fee schedules and rules govern billing.

Questions to resolve

  • Was the claim received and accepted for processing?
  • What specifically prevents payment or final processing?
  • Is a corrected claim, reconsideration or appeal the right route?

This is a general administrative checklist, not payer policy or advice about a particular patient. Confirm the current plan instructions.
