Medicine availability and current eligibility vary. For FARXIGA, AZ&Me stopped new enrollment May 1, 2026. Currently enrolled FARXIGA patients may receive assistance through December 31, 2026; the final refill-request date is December 15, 2026, with no extensions. Patients and providers may call this number for program information and refill questions. Separate annual re-enrollment rules for other medicines do not reopen FARXIGA enrollment.
