Completed Boehringer Cares enrollment application; the applicable current eligibility rules still govern.

Effective October 15, 2026, the program no longer accepts new Jardiance enrollments for people with Medicare Part D or Medicare Advantage prescription-drug coverage. Existing Medicare participants enrolled as of that date may continue through their current enrollment period and may be considered for 2027 only after annual re-enrollment, subject to eligibility. Uninsured applicants remain subject to current medicine, income, residence and outpatient requirements. No individual eligibility is established here.
