From the FHCP Provider Referral Guide (Seminole County, September 2026) key contacts table. Florida Health Care Plans is a Daytona Beach–based HMO (commercial and FHCP Medicare).

Fax the FHCP prior authorization/referral form with supporting clinical documentation. URGENT requests must be called in before submission. Routine determinations may take up to 7 calendar days (Medicare) or 14 (other lines). Non-urgent services should not be scheduled until approval.

Toll-free alternates printed on the FHCP prior authorization form. The referral guide gives the toll-free phone with extension 3230 for Central Referrals.

Inpatient authorizations (including inpatient mental health) are handled by FHCP Utilization Review; the admitting facility contacts UR and faxes documentation. Not for outpatient referrals (use Central Referrals).

Claims customer service. FHCP newsletters note up-front claim rejections are returned through Availity. No paper claims mailing address was published in the reviewed guide.
