Healthfirst NY Provider Manual (updated 10/1/2026). Preferred route is the online authorization tool in Availity Essentials. The manual lists two different Medical Management/UM fax numbers (1-646-313-5603 in the department table and 1-646-313-4603 in the prior-authorization section), so no fax is given here; confirm the fax with Utilization Management before faxing PHI.
