Paper claims (payer ID 40026)

This is a self-funded plan administrator, so each employer plan differs: the provider number, claim address and payer ID printed on the member's ID card are authoritative. Claim submissions page: clearinghouses use Payer ID 40026 with 275 attachments enabled; registered TINs can submit online through Optum iEDI with payer ID 40026; mail paper claims to the address shown. The contact page also says to use the claim mailing address on the member's ID card; use it when it differs.
