Current linked June 2026 form provides this separate reimbursement question line and mailing destination. It requires the reimbursement form and specified insurance/card, prescription and receipt evidence. The program also discusses eligible generic antidepressants, but this batch does not infer or import a generic medicine roster. Reimbursement remains subject to review.
