Mailed records requests

Release of Information for TGH hospital-based inpatient/outpatient facilities (Davis Islands, Brooksville, Spring Hill, Crystal River and others). Form A503PT Rev. 5/1/2023 calls the fax a dedicated fax line for patients; TGH faxes records only to medical facilities for immediate care. No charge when records are faxed or mailed to a verified physician office.
