All bills and adjustments (faxed bills are not accepted)

L&I's billing page: all bills and adjustments are mailed to the P.O. box below; L&I does not accept faxed bills. Put the L&I claim number on every page. Reports and chart notes go to a separate address and fax. Workers' compensation: a claim number and the adjuster or injured-worker identification are usually required; coverage is determined by the claim (not by a member ID), and state fee schedules and rules govern billing.

Questions to resolve

  • Was the claim received and accepted for processing?
  • What specifically prevents payment or final processing?
  • Is a corrected claim, reconsideration or appeal the right route?

This is a general administrative checklist, not payer policy or advice about a particular patient. Confirm the current plan instructions.
