EMPLOYERS' contact page says to fax Requests for Authorization to this number using its RFA form. For California, the page directs providers to the DWC RFA form; for Texas and for Massachusetts/New York it lists separate utilization-review contacts (see Arbicare line). Texas fax not included because its published number could not be cross-checked. Workers' compensation: a claim number and the adjuster or injured-worker identification are usually required; coverage is determined by the claim (not by a member ID), and state fee schedules and rules govern billing.

EMPLOYERS' contact page says Massachusetts and New York authorization requests should be emailed to Arbicare (intake) rather than the RFA fax. Workers' compensation: a claim number and the adjuster or injured-worker identification are usually required; coverage is determined by the claim (not by a member ID), and state fee schedules and rules govern billing.

  • What is the case number and next status-check date?
