Nevada Medicaid Fee-for-Service transitioned to Magellan Medicaid Administration effective July 1, 2022; current Prime Nevada documents publish the supported PA route separately. The accessible FA-59 Optum form is dated May 11, 2017 and lists historical 855-455-3311 / 855-455-3303; these values are withheld from current routing. This does not establish disconnection and is not based on Optum’s general fax-retirement notice. Do not apply FFS routing to managed-care pharmacy benefits without checking the plan.

Peoples Health’s current coverage-decisions page explicitly directs doctors/providers to Optum Rx at 800-711-4555 for Part D coverage-determination requests. Louisiana plan scope is supported by its October 2025 fact sheet; no nationwide plan availability inferred. The current plan page does not publish hours for this PA number, so the former 7 a.m.–midnight/Saturday hours are withheld. Optum’s general PA webpage separately publishes weekday Pacific hours, which are not promoted as Peoples Health-specific hours.
