State Farm says that if a clearinghouse or billing vendor is unsure how to route electronic bills, it should contact the State Farm claims department at this address with the claim number. For routing/payer-ID questions; not for clinical or payment decisions. Auto medical payments/PIP: a claim number and the claim adjuster or policyholder identification are usually required; coverage is determined by the claim and policy (not by a member ID), and state PIP fee schedules and rules govern billing.
