Paper medical bills (include TIN and claim number)

SAIF's billing page: send medical bills to the Salem address, include the TIN and claim number, and bill within 60 days of the date of service. Billing or payment questions (and provider address, tax ID or W-9 changes) go to providerquestions@saif.com or 503.315.3315. Payment follows the Oregon fee schedule or MCO contract rate. Workers' compensation: a claim number and the adjuster or injured-worker identification are usually required; coverage is determined by the claim (not by a member ID), and state fee schedules and rules govern billing.

Questions to resolve

  • Was the claim received and accepted for processing?

  • Is a corrected claim, reconsideration or appeal the right route?

This is a general administrative checklist, not payer policy or advice about a particular patient. Confirm the current plan instructions.
