Signed records requests; subpoenas per page
Patients send the signed UF Health medical request form by fax, mail or in person (UF Health Professional Park, 3300 SW Williston Road, Gainesville). UF Health generally does not fax records except to providers.
For non-UF Health Shands doctors' offices: use the office fax cover sheet with patient name, DOB and records needed, write STAT, and fax to ROI; ROI faxes records directly to the office. Same fax printed on the UF Health authorization form (RI0001).
