Paper medical bills

Pinnacol's provider page: paper bills (for providers not required to bill electronically) go to the Clinton, Iowa P.O. box, this fax, or the bill-submission email. First-time billers must also fax a W-9. Email bills as PDF or other standard documents, not PNG. Electronic billing is required for providers and SelectNet participants submitting 25 or more bills a month. Workers' compensation: a claim number and the adjuster or injured-worker identification are usually required; coverage is determined by the claim (not by a member ID), and state fee schedules and rules govern billing.
